You can download and complete the PGx Test Requisition Form by visiting our ‘Ordering The PGx Test‘ webpage. The completed form can be provided to us on the day of sample collection.
You can pay for the test on the day of sample collection by choosing one of the three secure payment methods provided on the PGx Test Requisition Form.
We will collect your sample for Free from the medical center, clinic, or hospital you plan to visit, or from your home or office at your convenience.
We will drop your sample at our NATA-accredited lab on the same day for testing.
We will send your test report directly to your referring medical professional within 3-5 days, who will use the report, along with other relevant information, to recommend medication and prescribe the most suitable course of treatment for you.
To ensure accurate sample collection, we kindly ask you to refrain from consuming any food or drinks 30 minutes prior to the appointment.
